California Consumer Rights Request Form

The California Consumer Privacy Act (CCPA) provides California residents with enhanced consumer privacy rights. If you are a California resident, please use this form to exercise your CCPA rights.

You can select one of the following CCPA rights under "Type of Request":

 • Right to Know: Know the categories and specific pieces of personal information we collected about you.

 • Right to Delete: Have us delete some or all of your personal information.

 • Do Not Sell My Personal Information: Request to opt-out of future personal information sales.

If you are an authorized Agent making a request on behalf of a California consumer, click here.

All disclosures will cover the prior 12 months from the time we receive your request. If the information you provided matches our records, we will respond to your request within 45 days. If more time is needed, we will notify you about why we need asn extension and a timeframe when you can expect us to respond (up to an additional 45 days). All "Right to Know" and "Right to Delete" requests are subject to certain exemptions under the CCPA. Please note you can submit no more than two California Consumer Rights Requests in a 12-month period.
